Welcome to the UOP centre for Senior's Health and Wellness. Are you looking for a place where people will remember your name, know your medical history and look out for your well-being? Our caring staff work together to make sure you are looked after whether it is for a chronic problem, a recent surgery or a new injury. Collectively we have over 50 years experience treating seniors. Though our therapists work with people of all ages, a significant portion of the client population is comprised of elderly people. Our experience working with this age group has kept us sensitive to the issues facing senior citizens, their complex health needs and the factors that influence comfort and quality of life.

Our OBJECTIVES

- To learn from, educate and work together with the staff to sustain the residents' level of functioning and thereby enhance their life-style.
- To utilize volunteers to enrich programming and, to invite the community as active participants in the programs through individual volunteer efforts and organized group programs.
- To demonstrate the value of publicly funded rehabilitation services to the community, municipality and government through the use of outcome measures and resident input.
Our RESOURCES
- Team
UOP has a talented and dedicated multidisciplinary team of healthcare professionals. We will provide your residents with their own Physiotherapist and the appropriate support staff to meet the identified needs at your facility. We work collectively with the in-house teams to complement and enhance programming.
- Equipment
The clinic will provide the necessary physiotherapeutic equipment.
- Skills & Education
Physiotherapists are recognized under the Registered Healthcare Professionals Act of Ontario. The Physiotherapists at UOP are all registered with the College of Physiotherapists of Ontario (CPO).
The Physiotherapy/Occupational Therapy Assistants at UOP are all graduates of accredited college programs. They are required to have Basic First Aid, CPR certification, a Police Background check and to participate in continuing education opportunities throughout the year.
The Kinesiologists are all members of the Certified Kinesiology Association and are also required to complete continuing education credits to maintain their membership. They are also certified in Basic First Aid and CPR.
Rehabilitation and Wellness Programs & Services
Our experienced professionals are committed to providing the highest level of rehabilitative care to your residents. Residents are individually assessed by a Physiotherapist to determine their admission level of function, the immediate rehabilitation needs and the goals of the resident.
Programs are tailored to individual functional abilities and can be provided in either group or private format. We provide a variety of treatments and programs that include:
- Hand Therapy
Hand pain and decreased hand strength continue to be two of the most commonly reported symptoms from our residents. Pain in the hands is a factor in functional decline and decreased independence. Pain control, strengthening and stretching for the hands can help residents to maintain functional independence and supports a better quality of life. - Mobility Program
Our mobility and ambulation program is geared toward the individual needs of the resident. Using standardized outcome measures we are able to track progression and decline in resident's status. We provide residents with training and practice in the use of assistive devices, wheelchair mobility and transfers. We closely monitor and progress residents who return from hospital after surgery, illness and/or fractures. - Falls Prevention Program
- Group Programs
To promote both social interaction and general well-being, group exercise classes are scheduled at different times throughout the week to accommodate residents. Group classes target strengthening, coordination, range of motion and endurance all within the abilities of the residents. Balance exercise classes are conducted for residents that display higher levels of function for ongoing maintenance. They are also geared toward residents that have injuries or diseases that affect their balance systems. - Individual Program
This aspect of programming often involves individualized exercise programs, comfort or pain control through the use of Physiotherapeutic modalities and manual therapy techniques. - Inservice Education
As life-long learners, our Health Care Professionals are committed to health promotion and injury prevention. We offer inservices on relevant topics for in-house staff, residents and families. Some of topics include: Arthritis, Osteoporosis, Safe Worksite Handling Techniques, Pilates, Hand Therapy, The Shoulder after Stroke, Chest Physio, How to Avoid Falls, etc.
PROCEDURES
- Charting: In accordance with the College of Physiotherapist of Ontario guidelines, UOP will keep its own charts to be completed monthly and will complete in-house charting quarterly. Either the therapist or the assistant will do charting. Each note will indicate the current treatment, outcome measure scores, short and long term goals, and any change in the resident's status.
- Frequency of Treatment: Frequency will be determined according to the assessment, the resident's status, and the diagnosis. In most cases, treatment will be provided two to three times per week.
- Reimbursement: As UOP is licensed by OHIP, your facility will not be responsible for direct reimbursement. The Facility will be required to provide UOP with the referring doctor's name, as well as the date of birth and health card numbers of all residents that receive assessment and treatment.
- Communication: Open communication between UOP and your team will be maintained to discuss ideas, to identify problems and to resolve issues as they arise.
